紅楓灣APP：在第六屆BHIVA/BASHH聯合會議上，利物浦大學研究團隊公布的一項回顧性研究顯示，doxyPEP在臨床預防梅毒和衣原體感染方面總體安全有效，抗生素耐藥風險增加并未形成新的公共衛(wèi)生危機。
DoxyPEP是一種預防性傳播感染的新策略，包括在性行為后72小時內服用一劑多西環(huán)素（常見且耐受性良好的抗生素）來降低梅毒和衣原體感染的風險。
研究方法
究由利物浦大學NHS信托的Martyn Wood團隊開展，分析了自2024年3月起利物浦地區(qū)（淋病感染率位居全國前列）所有培養(yǎng)陽性的淋病樣本，在這1416例樣本中有875例采自doxyPEP推廣前，541例采自推廣后，并對不同抗生素的敏感性進行了對比分析。
研究結果
doxyPEP推廣并未導致新的、嚴重的耐藥危機。
·推廣doxyPEP后，淋病軍對環(huán)丙沙星耐藥率從38%上升至47%，對四環(huán)素耐藥率從74%升至90%，但這種上升僅恢復到全國平均水平（環(huán)丙沙星46%，四環(huán)素91%），并未出現超出背景水平的異常增長。
·阿奇霉素和青霉素的耐藥率無顯著變化，且在所有樣本中均未發(fā)現對頭孢曲松（目前治療淋病的重要一線藥物）產生耐藥的菌株。
·耐藥增加主要發(fā)生在直腸和尿道取樣位點，與是否接受doxyPEP，以及性別或性取向無關。
臨床意義
研究者也提醒，這類觀察性數據無法證明因果關系，但結果總體支持在更廣泛人群中推廣doxyPEP。
此外，會議上展示一份海報也顯示，在男同性戀、雙性戀男性及與男性發(fā)生性行為的跨性別女性中，doxyPEP有減少梅毒和衣原體感染的潛力，進一步支持其作為公共衛(wèi)生干預工具的價值。
